Everything works when that plugin is turned off?Please confirm if I understand correctly and add FTP details and I will get a developer to have a look if the issue is with Popup Maker or UsersWP.

Can you post URL and WP admin details in a private reply so we can check.
My tests show that the profile pic should be used everywhere like you say, but maybe it is theme dependent.

Using a reCaptcha will stop the bots, but you never can stop desperate people.
