No Login Fields. No Registration Fields!?

Am using js composer with geodirectory. Have disabled all plugins including captcha plugin for both geodirectory and userwp. Have looked at the inspector. Am at a loss at this point. Login form (http://prntscr.com/ocsypu) and registration page (http://prntscr.com/ocsz0w) show nothing in the way of fields to fill out.

In most cases if you uninstall (and delete) the plugin, the add and activate it again fixes this issue.
If not please share WP Admin and FTP in a private reply and we will find a solution.
