There seems to be a problem with the theme and that plugin “Class ‘Layers_Form_Elements’ not found”

It suggests one of them is out of date: http://docs.layerswp.com/doc/layers-pro-cannot-activate-fatal-error-class-not-found-in-layers-pro-extension/
