Registration / Signup page not working urgent

We have moved to a support ticketing system and our forums are now closed.

    Hello. I just signed up for the pro version and thank you for your plugin.

    Regarding Registration… On the site https://usfdins.com the “SignUp” page is not working. usfdins.com/signup/ The form itself is not on the page even though in the admin dashboard I have selected /signup/ as the page. (screen shot attached)

    I did not. I assumed that selecting the /signup/ page would auto insert the code. Where do I find the short code please.
